Community Outreach Team (CREST)

The Community Outreach Team (CREST) focuses on raising public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals, provides emotional support to individuals and their caregivers and links individuals to relevant health and social care services when necessary. 

- Conduct outreach events to raise awareness on mental health and dementia within the service areas. Examples include dementia and mental health awareness activities to the public and community partners, mood and memory screening, door-to-door outreach etc. 

- Establish a seamless care continuum with Primary, Health and Community Care Partners 

- Promote and support Dementia Friendly Initiatives (DFIs) 

– Identify and highlight opportunities and gaps in the community, to create an inclusive environment for persons with dementia and their caregivers.
