Community Intervention Team (COMIT)

The Community Intervention Team (COMIT) provides assessment, counselling, therapy, case management and psychoeducation support for clients with mental health issues and dementia, including their caregivers. COMIT works closely with the community outreach teams, General Practitioners, Polyclinics and other community partners to provide holistic care to clients.

COMIT conducts needs assessments, develops individualised care plans and provides counselling and other forms of psychosocial intervention services such as psychoeducation and case management for clients and their caregivers, based on their care needs.

Besides interventions, the team conducts awareness training and community outreach engagements.

COMIT is community-based thus, we work closely with healthcare providers and other community and social care partners in the management of clients with mild to moderate mental health conditions, within the primary care setting.

COMIT covers clients aged 18 years and above. (People with mental health issues aged between 18 to 20 years old must obtain parental or guardian’s written consent).

They must be Singapore citizens or Singapore permanent residents (approval must be sought from MOH for foreigners or work permit holders). They should not be actively suicidal or violent.
