The Community Intervention Team (COMIT) provides assessment, counselling, therapy, case management and psychoeducation support for clients with mental health issues and dementia, including their caregivers. COMIT works closely with the community outreach teams, General Practitioners, Polyclinics and other community partners to provide holistic care to clients based on their care needs.

COMIT is community-based thus, we work closely with healthcare providers and other community and social care partners in the management of clients with mild to moderate mental health conditions, within the primary care setting. 

COMIT serves clients aged 18 and older. For individuals aged 18 to 20 with mental health concerns, written consent from a parent or guardian is required.

Individuals must be Singapore citizens or Singapore permanent residents. For foreigners or work permit holders, prior approval from the Ministry of Health (MOH) is required. Additionally, individuals should not be exhibiting active suicidal tendencies or violent behavior.

Besides interventions, the team conduct awareness training and engages in community outreach activities.
